Step 3 – Enter some basic info about your quiz Enter your quiz name and a short description for your quiz. You also could set th quiz language and visibility to the quiz. You are allowed to upload a picture for the quiz to make your quiz more popular if you like. And then, click Next to advance to the quiz result setting page.
For this frame you need one Question and various Anwers (all static text fields), an invisible (no border/background) Comment (dynamic/input text field) and one Button for every answer you offer. Give a name to the variable in the input text field and than simply put this code into button with right answer:

There are two ways to make quizzes and track results in Moodle, one way is create quizzes by third-party quizzes like QuizCreator, and another way is use the Moodle Quiz Module. As I wrote the article of how to make online quizzes in Moodle, today I write a tutorial of how to make a quiz through Flash quiz tool and upload to Moodle with SCORM compliance.
The quiz template is included in Flash MX and above versions, and it is designed to automatically track results and send them to a learning management system configured for your quiz. Furthermore, it should be SCORM and AICC compliant for using in Learning Management System (LMS).
