Select brush tool and apply it over the mouth. Use dark shades of red.
Select the Dodge Tool and change the Exposure to about 20%. Select a Soft Round brush and make it smaller than the space around the pupil. Click and drag the cursor around a several times to get the desired pale-eyes-effect.
I was fortunate enough to find a pic of this distinguished gentleman already in black and white. If you're working in a color photograph go ahead and take out the color by going to Image>Adjustments>Desaturate.
