These were great videos. One question. Will this work with Process Automation, specifically the Change or Incident Management Rule sets? I would like to have emails sent to the members of the queues when new requests are added/changed/etc. Or does one build them into the work flow itself?

I just tried reproducing your steps twice and and the following have occured:
At the Submitted Confirmation I just get a white screen with a process message. Not the Thank You...Your request has been successfully submitted.
The ticket is generated but under status it says Exception
When I try to work the ticket, there is nothing under the Actions menu.
I am currently running WorkFlow 7.5. Is there any known issues with the template and this version.

Is it possible to create our own ServiceCatalog.Request.Template.package?
I would like to start from the one available for download here, and modify the forms to meet our company's formatting.

This can be done with the ServiceCatalog template or any other Workflow project. All you simply have to do is open the project in question. Make the changes. Repackage the project. Copy the .package file into %programfiles%\symantec\workflow\designer\templates.
