Note:  If you download a PDF fillable form, you should "lock" the PDF document after filling it out in order to prevent users from manipulating your information.  CM/ECF will prevent users from making permanent changes to the original document, but the form fields will remain active if the document is not locked.  To lock your form and save your changes, do not use the "Print" or "Save" buttons on the fillable form. Instead click on the "File" menu in Adobe Acrobat, and then select "Print."  Choose your PDF writer from the Printer Name list and then click on Print or OK depending on your program.
