    Contact your child's healthcare provider or the county health department for a record of all the immunizations your child has received.

    Know all the vaccinations your state requires for elementary school attendance. The general age guidelines for vaccines are as follows:
    • 4 doses of Diphtheria Tetanus and Pertussis DTaP (DTaP) are required, with the last dose given around the child's 4th birthday. This vaccine protects against tetanus, diphtheria and pertussis (whooping cough).
    • The Hepatitis B vaccines are required before attending school. This consists of 3 doses, 1 given within hours after birth, with the last one after 24 weeks.
    • 2 doses of the MMR (Measles, Mumps,Rubella) vaccine. The first dose should be given at 1 year or older. The second dose should be given within a month of the first.
    • 2 doses of the Varicella (Chicken Pox) vaccine should be given before a child enters kindergarten. The first dose should be given after a year old.
    • Your doctor can substitute a record of chicken pox disease in place of the vaccine if your child already had the virus.
