This form is used to request the setup of a HSPH computer. It must be filled out completely and an email will be generated to Helpdesk. Also please note that new hardware should not be taken out of the box or started prior to the technician’s arrival. Upon receipt of your completed form the Helpdesk will contact you to schedule the setup.
