Allow staff to manually pick a matches map(s)

Currently each match contains a map_pool which contains map choice objects, the only way to pick maps is to have them randomly generated by staff by clicking the randomly generate button.
A simple form to specify the map choice objects from the given map pool is all thats needed, then save this form info in match.maps.
