Policy Puzzle 4th Edition: Questionnaires

The questionnaires are part of the first phase of data collection for the 4th edition of the Diabetes: Policy Puzzle to be published in November 2014. They aim to collect information on the state of the diabetes epidemic and related policies from 3 different types of national stakeholders:

• Patient organisations
• Healthcare professionals organisations
• National health authorities

Respondents are invited to answer in the name of the organisation they represent. Only the name of the organisation will be listed as a “consulted source”.

The questionnaires are available in 22 languages. We invite you to complete the questionnaires preferably online via one of the links below by 18 April 2014 latest.
